Assumption of the Risk and Waiver Statement
In consideration for the acceptance of my registration as a participant in the FrontRunner Training Group, and with the understanding that my participation in the FrontRunner Training Group is only on condition that I enter into this agreement, for myself, my heirs and assignees, I hereby assume the inherent and extraordinary risks involved in the FrontRunner Training Group and any risks inherent in any other activities connected with this training group in which I may voluntarily participate. I expressly assume the risk of and accept full responsibility for any and all injuries, including death and accidents which may occur as a result of my participation in the FrontRunner Training Group and release from liability, FrontRunner and each of their, representatives, employees and members. I hereby waive any claim I may have hereafter as a result of my participation in the FrontRunner Training Group and in any other activities connected with this training group in which I may voluntarily participate. I hereby agree to indemnify FrontRunner and its employees/representatives for any damages to myself or my property which may occur as a result of my voluntary participation in the FrontRunner Training Group. “I have read and understand the above statement.”
I give FrontRunner the absolute right and permission to use my photograph(s) in its promotional materials and publicity efforts. I understand that the photograph(s) may be used in a publication, print ad, direct-mail piece, electronic media (e.g., DVD, video, Internet) or other form of promotion or information. I release FrontRunner, its agents, staff, and the photographer from liability for any violation of any personal or proprietary right I may have in connection with such use.
